You will manage our Renfrewshire Community Link Worker service ensuring that a specialist, person centred service is delivered by a team of link workers to people who have health, social care or wellbeing needs.
The link workers are based within GP practices in Renfrewshire and you will be expected to manage all aspects of the programme delivery locally; including relationships with the Health and Social Care Partnership, GP and Practice Staff and wider community stakeholders.
The Link Worker programme is three fold - patient facing, practice facing and community facing - supporting, identifying and developing connections between patients, practice staff and local community providers is crucial to the programme, and to the role of Community Link Work Manager.
What we’re looking for
The successful candidate will have day-to-day responsibility for the leadership, management and governance of the project to ensure the delivery of a high quality service to our service users. He/she will also be expected to work collaboratively with project funders, relevant stakeholders and other local partners to build strong relationships and ensure effective working partnerships and practices.
We are looking for an individual who enjoys innovation, who is resourceful and who is attuned to the shifting landscape of health and social care provision, including digital developments to support health and wellbeing.
The role requires excellent leadership and team working skills and a sound knowledge of the social, economic and other factors which impact on the health and wellbeing of individuals and communities. Being able to effectively manage risk and safeguard is also a key aspect of this role. The capacity to work with competing demands and tight timescales will be underpinned by a commitment to quality.
In addition, applicants should have at least 2 years management experience in the health or social care sector and hold an SVQ 4 Health & Social Care (Adults) at SCQF level 9 certification or equivalent qualifications that are in line with SSSC requirements.
