You will be based in a GP practice within Prestonpans or Port Seton, providing a specialist, person centred service to people who have health, social care or wellbeing needs within that geographical area.
The aim of the Link Worker is to support people to identify barriers to their overall wellbeing and link them with the appropriate tools, community support or agency to overcome these. This will be delivered using a Social Prescribing model.
The Link Worker role is three fold - patient facing, practice facing and community facing - developing connections between patients, practice staff and local community providers is crucial to the programme.
What we are looking for
If you have a passion for supporting individuals to make sustainable change, have a sound knowledge of the East Lothian area and the social, economic and other factors which may impact on the health and wellbeing of individuals and communities, are qualified to SVQ Level 3 in Social Services and Health Care or Community Development or equivalent then we would love to hear from you.

This is a fixed term position for 12 months with the possibility of a further 12 month extension. There are two posts one 30 hours, the other 22.5 hours per week. Core hours of work will be Mon-Friday 9am -5pm.
